Instructions:

  1. Please type the information requested exactly as it should appear on the conference roster and participant's name badge.  If participant uses a nickname, please indicate how it should appear on name badge.
  2. Send completed form and fees payable in U.S. funds to Investment Recovery Association, P.O. Box 419264, Kansas City, MO  64141-6264.  Seminar registration fees are payable to: Investment Recovery Association. Payment must be received by August 1 to qualify for early registration.  Full refunds will be granted for cancellations received prior to August 1.  Cancellations received after that date will be subject to a 30% penalty.  NO REFUNDS will be made after August 13.  Registration fees cover conference functions and do not include accommodation payment.
